ALCHEMED LABORATORY DETAILS

As part of our project to facilitate the integration of your practice with various laboratories, we need you to to provide certain information and preferences. Laboratory details are collected to enable a push integration, which allows laboratory results to be sent automatically into your patients’ files within Alchemed. Please complete the details for your relevant laboratory or laboratories below.

If your practice has more than 1 Clinician number for a specified lab, please list both numbers in the input filed separated by a comma and a space. For Example, if your practice has two Ampath numbers  insert AMP123, AMP345

Also, for individual results to be pushed per patient, the patient ID number must be recorded within Alchemed.
